Our pledge
Embrace Psychiatry (a trade name of Legion Medical, LLC) is required by law to maintain the privacy of your protected health information ("PHI"), provide you with this notice, and follow its terms.
How we may use and disclose your PHI without authorization
- Treatment: coordinating your care, including with other providers, pharmacies, and labs.
- Payment: to obtain payment for services, including providing superbills you submit to your insurer.
- Healthcare operations: quality assessment, training, compliance.
- As required by law: public health reporting, court orders, prevention of serious threats to health or safety, mandatory reporting (abuse, certain communicable diseases).
Uses that require your authorization
We will obtain your written authorization before any use or disclosure not described above, including most uses of psychotherapy notes and any marketing communications. You may revoke an authorization in writing at any time.
Your rights
- Inspect and copy your medical record.
- Request amendments to your record.
- Request an accounting of certain disclosures.
- Request restrictions on uses and disclosures (we are not required to agree, except for disclosures to a health plan when you have paid in full out of pocket).
- Request confidential communications by alternative means or at alternative locations.
- Receive a paper copy of this notice.
- Be notified in the event of a breach of unsecured PHI.
Telehealth
Telehealth visits are conducted over a HIPAA-compliant video platform. While we use industry-standard safeguards, no electronic transmission is 100% secure.
Complaints
If you believe your privacy rights have been violated, contact us at hello@embracepsychiatry.com, or file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate for filing a complaint.
Changes to this notice
We reserve the right to change this notice at any time. The revised notice will be posted on this page and effective for all PHI we maintain.
